Birth/ Death/Marriage Certificates Information and Fees

In order to protect confidential information, there are a number of state regulations pertaining to who may obtain death and birth certificates.

By law, the town clerk can only issue certified copies of vital records.  EFFECTIVE OCTOBER 1, 2009, the fee for a full certified copy is   $20.00. a wallet sized copy of a birth record, which contains only name, date and town of birth may be obtained for  $15.00.  We accept cash or personal checks made out to Columbia Town Clerk

If you would like a certified copy of a vital record, you will need to complete a copy request form and present a copy of your Photo ID.

 

Puerto Rico's New Birth Certificate Law

The government of Puerto Rico has enacted a new law aimed at strengthening the issuance and usage of birth certificates to combat fraud and protect the identity and credit of all people born in Puerto Rico.

Under the new law, all Puerto Rico birth certificates issued before July 1, 2010, will be invalidated so that new, more secure certificates can be issued.

Those United States residents born in Puerto Rico will be able to request and receive their new birth certificates through a simple process by mail starting July 1, 2010.
